How to set up your appointment with a dietitian

Ask your provider for a nutrition referral.  If you see a provider outside CHI Health, you can locate the referral forms below.

We encourage you to call your insurance to check on coverage of medical nutrition therapy ahead of your appointment.  If your insurance does not cover this type of visit, you may qualify to use MD Save.  Learn more about MD Save and if you qualify. HSA dollars can be used for voucher purchase; provider referral is required for appointment. See all MD Save options.
