Home Care
Home Care provides individualized skilled care for patients, in the comfort of their own home. CHI Health at Home will work closely with the patient and their physician to coordinate services and develop an individualized plan of care. Our skilled nursing, physical, occupational and speech therapy, and personal care services improve lives by providing care for our patients wherever they call home.
Hospice Care
CHI Health at Home is honored to offer hospice services that provide compassionate end-of-life care for patients - no matter where they call home. As part of our commitment and dedication to serve through our ministry, we also play a pivotal role in supporting the patient's family and caregivers during this challenging time.
Founded on the belief that every person has the right to die pain-free and with dignity, our hospice care includes specialized pain control and symptom management by a highly skilled hospice team. At a time when exceptional care is needed most, CHI Health at Home employs a team approach to end-of-life care.
Home Respiratory Care
CHI Health at Home provides unique and specialized respiratory treatments and therapies by offering a full line of services, expert care, and access to high quality medical equipment and supplies for patients in metro Omaha and southwest Iowa.

Home Infusion Pharmacy
Home Infusion therapy services offer our patients a comprehensive and innovative source of health care options, in the comfort of their home. Alleviating long and unnecessary stays in the hospital, our home infusion team administers intravenous (IV) therapy and treatment in a safe and comfortable setting.
Nursing Network
The CHI Health Nursing Home Network is an efficient network of nursing care by a team of Nurse Practitioners that compliments and coordinates with the care patients receive from their skilled nursing facility. Our focus is to promote quality and function for the patient, as well, as support the family and skilled nursing staff.
Faith Community Nursing Network
The Faith Community Nursing Network leverages the resources of CHI Health and the strength of faith communities to educate, advocate and motivate people to take positive action regarding their mind, body and spirit.

Benefit Assistance
CHI Health at Home services are provided to the insured and un-insured alike. As an approved provider for most insurance companies in Nebraska and Iowa, we work with patients and their families to explain benefits and verify coverage before services begin — billing your insurance company directly to simplify the process and reduce your paperwork.
Convenient Referrals
Referrals or requests for information may be made by physicians, patients, family members, friends, clergy and other members of the community. We offer two methods of referral: phone or fax.
