Educational Services
Rehabilitation is a process of learning physically and mentally. To ensure long-term success, education should be an integral part of the rehabilitative therapy program. The education process begins from the first day a patient enters our facility. By educating both the patient and family, we make the transition from CHI Health Rehabiliation Care to home easier. Education ensures the continuity of the recovery process.
The Family is Involved in Therapy
The family of a patient plays an important role in the process of rehabilitation. They provide emotional and other support to the patient. And, they are often charged with facilitating continued rehabilitation after the patient is dismissed from an inpatient care facility. The rehabilitation center should involve family members in the care plan and provide support services that help them cope with the situation.
At CHI Health Rehabiliation Care family members are informed of progress at all stages. Family members are encouraged to be actively involved in planning and delivering the patient's physical therapy. Staff members are available to provide support to family members. We offer free parking and help to arrange overnight accommodations.
Ease of Access
While the type of care available is important, so too, is the accessibility of services. It may help to select a center that provides both inpatient and outpatient services, or one that has multiple locations that are convenient. CHI Health Rehabiliation Care is located on the Immanuel campus, which off offers inpatient, outpatient and subacute services. We also have several sites located throughout the Omaha area for outpatient therapy services.
Customized Assistive Devices
Even after therapy, patients may need assistive devices to help them function independently outside the rehabilitation center. The rehabilitation center should offer assistance in fitting these devices and their use. Equipment within the center that simulates real-life situations, such as a trip to the store or bank, can help the patient return to community life.
At CHI Health Rehabilitation Center, we provide assistance with the acquisition and use of assistive devices such as walkers, wheelchairs, prosthetic devices, etc. We simulate real-life situations to better prepare the patient to function independently at home or work. We make recommendations on home, work and car modifications and equipment to improve the patient's ability to function effectively.
Follow-up Care
Even after the acute physical rehabilitation process is complete, there are other steps that must be taken to integrate the patient back into the community. The rehabilitation center should provide some form of follow-up care such as support groups, job training, vocational counseling, outpatient therapy and more. We coordinate follow-up services with other agencies in the patient's hometown. CHI Health Rehabiliation Care patients have access to a wide range of follow-up care including: outpatient therapy, vocational counseling, job training, educational services and counseling.
Emergency Medical Care
Medical emergencies can happen anywhere. That is why it is important that the rehabilitation center have access to emergency medical care. And, its personnel should have training that lets them know how to respond in the case of an emergency. At CHI Health Rehabiliation Care our staff is fully trained to respond to a medical emergency. If additional care is required, we are located adjacent to CHI Health Immanuel for immediate access to emergency medical personnel and services.
