Adults 55+ Partial Hospitalization Program

The Heritage Partial Hospitalization Program is a short-term, ambulatory, active treatment program designed for adult (55+) mental health patients who are in acute psychiatric distress or have just completed an episode of acute inpatient care – but don’t require 24-hour supervision.

Located on the CHI Health Immanuel campus, we provide psychiatric crisis stabilization through daily, intensive programming. Individuals receive behavioral treatment in an outpatient setting while continuing to live in their own home or usual community setting.

Each patient referred is screened to determine the appropriateness of partial hospitalization services. Patients may participate if they meet criteria, are able to function independently and plan to actively participate in the program.

Adults 55+ Partial Hospitalization Program Specifics

The Heritage Partial Hospitalization Program is a five-day-a-week program, Monday through Friday. Upon admission, patients and staff members together develop an individualized, interdisciplinary treatment plan. The program includes:

  • Evaluation and assessment
  • Medication supervision and education
  • Health education
  • Group therapy

Respectful, compassionate and individualized care is the focus of our team, which includes:

  • Physician
  • Nurses
  • Mental health therapists
  • Chaplain

When the patient has met treatment goals and no longer needs the Partial level of care, follow-up outpatient care will be arranged.

Partial Hospitalization is appropriate for patients with:

  • Persistent feelings of sadness or anger
  • Social withdrawal/isolation
  • Agitation or restlessness
  • Confusion/disorientation
  • Loss of energy and motivation
  • Sleeping and eating disturbances
  • Difficulties with behavioral medical regimen
  • Unsuccessful previous outpatient behavioral treatment
  • Depression
  • Anxiety disorders

Our services are sensitive to the multiple needs of our patients, including the use of trauma-informed approaches to treatment.

Referral information

Medicare Part B and many private insurance companies cover the Heritage Partial Hospitalization Program.
